- W2111567382 abstract "SummaryIn 332 femoropopliteal bypass reconstructions using ePTFE, we noted a primary 5-year patency rate of 53.1% ± 7.6 (PI = above-knee) and 47.6% ± 6.2 (PHI = below-knee). 118 secondary operations were performed: 75 bypass thrombectomies and 43 secondary bypass procedures. Using the saphenous vein for secondary bypass reconstruction the 3-year patency rate was much better (65.2% ± 11.2) than using ePTFE (23.4% ± 15.4). We were able to perform relatively more secondary bypass reconstructions in failed PI grafts (22/36) than in cases with failed PHI grafts (21/94). Therefore, we noted a better 5-year limb-salvage rate for the primary above- knee bypass procedure (PI 78.4% ± 5.6, PHI 66.3% ± 5.6). Our results demonstrate that the preservation of the saphenous vein and the preferential use of ePTFE grafts is an adequate method in above-knee femoropopliteal bypass surgery. However, the saphenous vein should be used in all cases with poor run-off and for most cases of below-knee reconstruction. As an exception, the primary use of thin- walled, ring-supported ePTFE grafts for below-knee femoropopliteal bypass is justified in elderly patients." @default.
