- W2112908897 abstract "For hypofractionated prostate treatment, the daily dose to the targets, the bladder and rectum are critical. This study compares the dosimetric results of the patient re-positioning, daily re-planning techniques, and a hybrid method that combines both techniques. Five prostate patients were retrospectively studied, each had 1 planning CT and 11 daily CBCTs. Structures of interest (SOI) include CTV (prostate and seminal vesicles), bladder and rectum. Three IGRT techniques (“Re-positioning”, “Re-plan”, and “Hybrid”) were applied on each CBCT. The “Re-positioning” technique corrects the patient positioning by re-aligning the patient based on the target soft-tissue matching. The “Re-plan” technique performs the re-optimization daily using the CBCT structures after initial re-alignment. The “Hybrid” technique tries to “re-use” all the previous plans before performing the daily re-planning. It attempts to find a best fit plan with the smallest PTV that provides sufficient coverage for the daily CTV, from the patient plan library. If no suitable plan is found, the “Re-plan” technique is performed and then the new plan is added to the library. The cumulative doses of the three IGRT techniques are comparable (<1% and <5% differences for CTV and bladder) in term of the CTV coverage and bladder sparing for all patients. The mean and D30 of the rectum cumulative doses in the “Re-positioning” plans are 5% and 10% higher than those of the “Re-plan” technique, while the “Re-Plan” and the “Hybrid” techniques show <2% difference. Daily bladder and rectum sparing is improved significantly with the “Hybrid” technique over the “Re-positioning” technique. For rectum, the average (min-max) daily D30 doses are 84% (67%-94%) and 70% (51%-78%) for the “Re-positioning” and “Hybrid” techniques, respectively. Similarly, for bladder, the averages (min-max) daily D30 doses are 57% (42%-75%) and 59% (46%-70%) for the “Re-positioning” and “Hybrid” techniques, respectively. Of the 11 daily fractions, the average number of the fractions that require the “Re-plan” for the “Hybrid” technique is 3.2 fractions for each patient. However, there is a wide range among patients, ranging 1-7 fractions. The hybrid technique retains critical structure sparing of the “Re-plan” technique, and reduces the number of fractions requiring re-plan.
