• W2113428139 abstract "In both preeclampsia and fetal growth restriction, maternal cardiac function deteriorates as disease progresses. Nulliparous women are at increased risk of preeclampsia, whereas multiparity is considered to be a risk factor for peripartum cardiomyopathy and also for cardiovascular disease later in life. This cross-sectional study, performed in the antenatal clinic of a teaching hospital, was planned to show whether maternal cardiac adaptations in the first trimester of pregnancy are associated with increasing parity. Two-dimensional echocardiography of the maternal left ventricle was performed at 11- to 14-week gestation using a 3.5-Mz transducer in 2352 parous women and 2337 who were nulliparous. Obstetricians who had been trained by cardiologists carried out the studies. Compared with nulliparous women, those who were parous had significantly higher median values for cardiac output (5.6 vs. 5.2 L/min) and cardiac index (2.3 vs. 2.1 L/min/m2). These differences were attributed to significantly higher values of median stroke volume, heart rate, and left ventricular outflow diameter, and lower values of total vascular resistance and median uterine artery pulsatility index. Mean arterial blood pressure did not differ significantly between nulliparous and parous women. All maternal cardiac variables increased progressively except for total peripheral resistance, which declined with increasing parity. Birth weights were higher in the parous group (3.39 vs. 3.23 kg), and were independently related to maternal hemodynamic variables and to demographic and social characteristics. These findings show that, compared with nulliparous women, pregnancy in parous women is associated with a higher maternal cardiac output and also with higher birth weight." @default.
