- W2113449823 abstract "Introduction: The AcrySof foldable acrylic intraocular lens (IOL) has become an accepted and frequent means of treating pediatric aphakia. The manufacturer recently introduced a new version of this IOL, the AcrySof Natural, with blue-light filtering properties purported to offer superior retinal protection. After using this lens for 10 months, it is our impression that it is generally well tolerated in children. Nonetheless, we encountered unusually high amounts of inflammation in some patients. Methods: A chart review identified 28 eyes of 21 children (age 1 month to 13 years) who had the AcrySof Natural IOL (type SN60AT; Alcon, Fort Worth, TX) implanted. Postoperative records were analyzed. Eyes with coexisting ocular pathology were excluded. Results: All surgeries were uneventful without intraoperative difficulty or complications. In 16 eyes (57%), no postoperative complications were noted. In 12 eyes (43%), however, significant amounts of postoperative inflammation including fibrinous membranes on the optic, posterior synechiae, significant cell and flare, and rapid development and recurrence of posterior capsular opacification were observed. These cases were treated with systemic and topical steroids and cycloplegic agents. One eye required explantation of the optic 5 months postoperatively due to a persistent inflammatory membrane surrounding the IOL. Discussion: Although we are generally pleased with our experience with the AcrySof Natural, the incidence of significant inflammation appears to be higher than with other acrylic IOLs (eg, AcrySof SA and MA). This finding is puzzling because the device is chemically very similar to the other models. We hypothesize that the antigenic properties of the Natural’s yellow chromophore or confounding factors such as packaging or contamination of the IOL may play a role." @default.
