For the most part, the selection of treatment for most diseases is dictated by physician recommendations and patient preferences and not based on cost. Payment for most medical care in the United States and countries with socialized medicine is primarily from the government or third-party payers that only have an indirect impact on medical decision makers. In the United States, Medicare dictates reimbursement for medical services, which can influence the choice of treatment to some extent. For example, decreasing compensation for procedures will result in the limitation of care to some degree. A 2001 survey by O’Leary and colleagues found that 16% of urologists discontinued tests or procedures due to decreased reimbursement [2]. Another study found that when Medicare fees for selected ‘‘overpriced’’ procedures and diagnostic tests were reduced, there was a decrease in the volume and complexity of the services provided [3]. These controls are not significant enough to contain costs to a large extent, and there has been an everincreasing burden of health care costs on governmental budgets. Moreover, there is a realization that the overall cost of medical care does not correlate directly with the overall quality of care. For example, the United States, which spends the most per capita on health care, does not rank highest in terms of health care outcomes based on World Health Organization statistics [4].
