- W2114405168 abstract "Image-guided needle biopsies are increasingly being utilized to guide clinical management of a variety of tumors, including renal cell carcinoma (RCC). Biopsy analysis is particularly important for small renal masses (< 3 cm), where distinguishing benign from malignant lesions is difficult based on imaging alone. Since routine morphologic examination of needle biopsies may be limited by small tissue quantities, low cellularity, or damaged architecture, we recently developed a classification decision tree based on the presence or absence of RCC subtype-specific genomic gain/loss and translocations as detected using fluorescence in situ hybridization (FISH). In an ongoing IRB-approved study of patients undergoing image-guided needle biopsies, we have found insufficient cells are obtained for some specimens for adequate FISH scoring across all 8 probe hybridizations in the decision tree. In the same study, we are evaluating the potential for RCC classification by array-comparative genomic hybridization (array-CGH) using a custom oligonucleotide array representing regions commonly altered in urogenital neoplasms. High-quality DNA (yield range: 200 ng-13 ug) has been obtained from all needle specimens to date, and all gave analyzable log ratio profiles. We have detected aberrations consistent with well-known copy number changes reported for RCC. In order to classify the specimens, an independent dataset of resected specimens with known morphology are being used to develop an RCC classification tree based on aberrations detected in each subtype. For the needle biopsy specimens then, a FISH- and array-CGH-based classification will be obtained and compared with that obtained by morphology alone. When available, the morphology of subsequently surgically resected renal masses will serve as the gold standard for classification. We anticipate that tumor subtype classification based on genomic gain and loss detected by array-CGH will improve our ability to accurately diagnose renal masses from needle biopsy samples." @default.
