Background: Since the prognosis of patients with insulin dependent diabetes mellitus is generally limited by secondary cardiac com- plications, an early determination of the myocardial manifestation is of considerable importance. The aim of this study was to investigate whether dobutamine stress echocardiography is a suitable method for the detection of early signs of diabetic cardiomyopathy. Patients and Methods: Using echocardiography (at rest and during dobutamine infusion), we examined 47 young patients with insulin dependent diabetes mel- litus (25 ± 3 years) and 33 age- and sex-matched sub- jects without cardiac disease and without diabetes mellitus. Using M-mode echocardiography morpho- logical parameters and systolic function were deter- mined. The parameters of LV diastolic function were assessed by Doppler echocardiographic analysis. Results: All type-I diabetics showed a normal systolic function and a delayed relaxation at rest. Under maximal dobutamine stress echocardio- graphy in seven patients a regional systolic wall mo- tion disorder was detected. These seven patients and nine further diabetics had a diastolic dysfunction in terms of a restrictive diastolic filling pattern at stress. In 14 patients with diabetes mellitus pseudonorma- lisation was documented and 17 patients showed an unchanged delayed relaxation at stress. Atrial and ventricular arrhythmias more often occurred in the 16 patients with the restrictive filling pattern (p < 0.01). Conclusion: In patients with insulin dependent dia- betes mellitus adrenergic cardiac stress reveals left ventricular diastolic dysfunction because of an in- crease of the heart rate and blood pressure. Patients with restrictive diastolic dysfunction particularly suffer from frequently occurring atrial arrhythmias. J Kardiol 2002; 9: 483-9." @default.
