- W2117125278 abstract "To analyze the profile, outcome, treatment related toxicities and its correlation to patient related and treatment related factors. One hundred and seventeen patients diagnosed to have pituitary adenoma and received radiotherapy between Jan 2004 and Dec 2010 were included in this cohort. Of these, 98 patients who had a minimum follow-up of six months were included for analysis retrospectively. Median follow-up period was 44.5 months with a range of 6 -235 months. Tests for correlation were performed for various patient factors and treatment related factors. Total number of patients included in the study was 98 (Mean Age - 41.7 years, Range 14 -71 years, 60.2% males and 39.8% females). Among all patients, 59.2% had nonfunctioning adenomas, 40.8% had functioning adenomas and 6% of patients had atypical features on histology. Surgery was done in 92 patients (82.6 % having had one prior surgery, 16.3% with two surgeries and 0.1% with three prior surgeries). Six patients did not have surgery. Indication for radiotherapy was residual in 65.3% patients, disease recurrence in 28.6% and 3 patients had reirradiation for the disease recurrence. Stereotactic radiotherapy technique was used for 68.4%, 3DCRT for 5% and conventional for 26.5% of patients. Radiotherapy dose delivered ranged from 45 - 57.6 Gy in 25 -31 fractions. The target doses were prescribed to the isocenter. The 95 % isodose encompassed the target volume. 35.7% of patients had received >50.4 Gy and 64.3 % of patients received ≤ 50.4 Gy. Radiological response was noted in 72.5 % patients (CR 16.3% PR 38.8 % SD 17.3 %); 23.5 % patients were not evaluable for radiological response; 4% of patients had disease progression. Biochemical response was noted in patients with functioning adenomas and they were found to have longer period of remission with radiotherapy. In functioning adenomas, discordance between radiological and biochemical response was noted. (Pearsons coffn: 0.041, NS) Post Radiotherapy visual deterioration was noted in 11 %(n = 11) of the patients and 4 % (n = 4) of patients had new onset hypopituitarism after radiotherapy. The total radiotherapy dose (>50.4 Gy) delivered was found to have correlation with the visual and endocrine complications. Radiotherapy is an effective treatment modality in the management of pituitary adenomas with 96% progression free survival in this study. In functioning adenomas, discordance between radiological and biochemical response was noted. Doses ≤ 50.4 Gy resulted in reduced visual and endocrine complications." @default.
