- W2118225853 abstract "The reports of allergic contact dermatitis are on the rise as a result of the increasing use of temporary tattoos. This letter highlights the complications that may arise from temporary tattoos and stresses the importance of informing patients of potential risks. A 67-year-old white man had 2 temporary black henna “tattoos” applied, one to the left side of his neck and another on the dorsal aspect of his left foot. Within 24 hours, he had pruritus, swelling, and redness develop at each of the tattoo sites and then had blisters and erosions develop on the dorsal aspect of his left foot. Three days after the tattoos were applied, he presented to the clinic. He was a healthy man with known allergies to sulfonamides and benzocaine resulting in a diffuse pruritic, erythematous dermatitis with the former and a localized, contact dermatitis with the latter. He had no history of hair-dye use or exposure. On examination, the left side of his neck showed a fading black-brown tattoo superimposed on an erythematous, edematous plaque (Fig 1). He also had a small number of excoriated, erythematous papules on the central aspect of his chest. The dorsal aspect of his left foot showed a 4- × 6-cm superficial erosion with a border of black-brown pigment, residual pigment from the original tattoo (Fig 2). Fig. 2Dorsal aspect of left foot with erosion, surrounding edema, and erythema at site of tattoo application.View Large Image Figure ViewerDownload Hi-res image Download (PPT)He was given a 2-week taper of prednisone, hydroxyzine, and topical mupirocin for the erosion on the dorsal aspect of his left foot. The reactions resolved completely without complications. If a patient has had previous exposure to PPD such as that found in hair dyes or to cross-reacting substances, induction times can be as short as 24 to 48 hours.1Mohamed M Nixon R Severe allergic contact dermatitis induced by paraphenylenediamine in paint-on temporary “tattoos.”.Australas J Dermatol. 2000; 41: 168-171Crossref PubMed Scopus (43) Google Scholar, 2Rubegni P Fimiani M De Aloe G Andreassi L Lichenoid reaction to temporary tattoo.Contact Dermatitis. 2000; 42: 117-118PubMed Google Scholar, 3Sidbury R Storrs FJ Pruritic eruption at the site of a temporary tattoo.Am J Contact Dermatitis. 2000; 11: 182-183Crossref PubMed Scopus (22) Google Scholar Reactions to PPD found in temporary tattoos are primarily type IV delayed-type hypersensitivity presenting as pustular dermatitis, eczematous and vesicular dermatitis, lichenoid reaction, or generalized dermatitis.1Mohamed M Nixon R Severe allergic contact dermatitis induced by paraphenylenediamine in paint-on temporary “tattoos.”.Australas J Dermatol. 2000; 41: 168-171Crossref PubMed Scopus (43) Google Scholar, 2Rubegni P Fimiani M De Aloe G Andreassi L Lichenoid reaction to temporary tattoo.Contact Dermatitis. 2000; 42: 117-118PubMed Google Scholar, 3Sidbury R Storrs FJ Pruritic eruption at the site of a temporary tattoo.Am J Contact Dermatitis. 2000; 11: 182-183Crossref PubMed Scopus (22) Google Scholar, 8Rietschel R Fowler Jr, J Fisher's contact dermatitis.in: 4th ed. PPD cross-reacts with related compounds that have an amino group in the paraposition of the benzene ring. Examples include para-amino benzoic acid, sulfonamides, sulfonylureas, dapsone, azo dyes, and benzocaine.1Mohamed M Nixon R Severe allergic contact dermatitis induced by paraphenylenediamine in paint-on temporary “tattoos.”.Australas J Dermatol. 2000; 41: 168-171Crossref PubMed Scopus (43) Google Scholar, 8Rietschel R Fowler Jr, J Fisher's contact dermatitis.in: 4th ed. Williams & Wilkins, Baltimore1995: 964-1055Google Scholar Although systemic sensitization is rare, topical sensitization with chemicals in the para-amino group have been shown to elicit systemic reactions.10Fisher AA Systemic contact dermatitis from orinase and diabenese in diabetics with para-amino hypersensitivity.Cutis. 1982; 29 (556,561-2): 551PubMed Google Scholar Given that the patient had a known allergy to sulfonamides and benzocaine, and no known exposure to PPD from hair dye, the rapid onset of his dermatitis was likely initiated as a cross-reaction to PPD in the tattoo dye. People with known reactions to PPD or cross-reacting allergens such as sulfonamides, para-amino benzoic acid, or benzocaine should be especially cautious of black henna tattoo application." @default.
