- W2118932689 abstract "The lead article in this issue is the Arthur Wilson Oration as delivered by Professor Alastair McLennan, who is well known both nationally and internationally for his many contributions to our speciality. His remarkable character equips him well for taking on difficult roles and I am sure he is the only person in the world who can claim to have been President of both a leading perinatal research organisation (PSANZ) and also the Menopause Society. His interests both academic and clinical have always been many and varied. His oration covers, from a historical perspective, many of the important changes in reproductive health and outcomes which have affected women’s health generally over the last 150 years. Alastair has brilliantly intertwined many themes including fertility rates, maternal mortality, cerebral palsy, menopause and even obesity. For those of you who did not have the pleasure of listening to the oration it is reproduced in full. Of course, Arthur Wilson is intrinsically linked to this journal. In 1959 the Arthur Wilson Memorial Foundation expressed an interest in sponsoring an Australian journal on obstetrics and gynaecology, and both the Australian and New Zealand Regional Councils of the Royal College of Obstetricians and Gynaecologists (RCOG) were receptive to the proposal. The publication was backed financially by the Arthur Wilson Memorial Foundation and an ANZ Journal Company inaugurated. In March 1961 the first issue appeared, with a circulation of about 500 copies. Almost 50 years on we have grown to an international circulation of over 5000. I hope that both Alastair and ANZJOG have proved to be worthy recipients of Arthur Wilson’s legacy. For this issue I have asked one of our Associate Editor’s, Peter Dietz, to showcase an article from the 1980s and I am very pleased that he has chosen one by one of my own teachers from the time when I was a trainee. He has selected a paper published in 1987 that had a substantial impact on Australian practice in Gynaecology, even though the underlying clinical question is still unresolved.1 Peter has written the following commentary. Stress urinary incontinence in women is treated surgically, in 1987 mostly by Burch colposuspension and fascial or synthetic urethral slings, and in 2010 mostly by mid- urethral synthetic slings. Then, just as now, practitioners have to decide on the extent of the diagnostic workup prior to surgery. Korda and colleagues argue that it is not safe to rely on history and examination alone, and that urodynamic testing is required to optimise treatment outcomes. Unfortunately even now, over 20 years later, there is no randomised controlled trial to test this hypothesis. A Cochrane review of the role of urodynamics in the management of urinary incontinence found no evidence for or against urodynamics in 2002, and the situation is unchanged today.2 The best evidence we have is indirect, showing that there is a substantial discrepancy between patient history and the results of urodynamic testing, and the Korda paper is one of the largest such series ever published. Expert opinion, based on such data, suggests that urodynamic testing be done prior to anti-incontinence surgery and the relevant RCOG guideline supports this stance.3" @default.
