In general, we are in the early stage with only minimal progress seen in HCV and HIV-1 testing. However, more technological advances in both the pre-analytical and analytical arenas may expedite this complex process. Specimen processing is a priority for some companies at the present time, with particular focus on automation and commercially available test reagents. Specimen processing is no doubt a roadblock in the path to standardization. Recently, we have seen the introduction of real-time PCR technology (TaqMan and LightCycler). This technology combines the amplification and detection steps, shortens the turnaround time, adds the option of quantitation (Figure 4), and essentially removes the contamination threat. In fact, the LightCycler even has been used to replace culture for Herpes simplex virus in one prominent clinical laboratory (10).
