- W2122150072 abstract "My grandmother was fond of reminding me of the ecclesiastical notion that there is a time for every purpose under the heavens, and in this issue of The Lancet the study by Nicole Smeets and colleagues is long overdue. Across the globe, physicians and their patients are confronted many times each year with the diagnosis of basal-cell carcinoma. Together, they must choose, often with incomplete knowledge, from various treatment options available for the eradication of tumours that carry several price tags and cosmetic implications. Too often the choice is driven by a lens that allows only a single focus, based on: opinion, training, cost, and cosmesis; self-interest on the part of physicians or patients; and concerns about recurrence or charges of malpractice. Ideally, the art of medicine allows a more precise vision seen through a prism that accounts for the many competing priorities, arriving at the best treatment for the individual patient. This ideal is close to fruition with Smeets and colleagues' report. Surgical excision vs Mohs' micrographic surgery for basal-cell carcinoma of the face: randomised controlled trialNo definitive conclusion on recurrence rates of primary or recurrent basal-cell carcinomas is yet possible. Although recurrence rates were lower after MMS than after SE, the differences were not significant. Full-Text PDF Department of ErrorAnon. Surgery for facial carcinomas. Lancet 2004; 364—In this Talking Point (Nov 13), the final sentence should read: “In a Comment paper, Phillip Williford and Steven Feldman question the cost of MMS, and say that although the procedure is appropriate for recurrent tumours, large facial tumours, and those with aggressive histological patterns, it should be used sparingly for less risky tumours on less critical body sites.” Full-Text PDF" @default.
