Voriconazole represents one of the most widely used drugs for prevention and treatment of IFI in patients after lung transplantation; however, its use may be complicated by adverse reactions, including photosensitivity skin reactions, as well as other systemic side effects such as confusion, fever, muscle pain, shortness of breath and agitation. It is hypothesized that that the most likely mechanism of phototoxicity is either related to voriconazole itself or to its main metabolite, the N-oxide voriconazole metabolite (MVRC), and an interaction with retinoid metabolism. 1 Thomas A.
