- W2123545875 abstract "Posterior cortical atrophy (PCA), a variant of Alzheimer's disease (AD), primarily affects posterior regions of the brain and is characterised by visuospatial and visuoperceptual deficits with relative sparing of memory. Previously we found that PCA subjects had significantly smaller hippocampal volumes than controls despite having relatively preserved memory. In this study, we investigated hippocampal shape differences in subjects with PCA (n=47), control subjects (n=48), and typical AD (tAD) (n=29). In addition to describing location of surface deformations of the hippocampus, shape information may give greater diagnostic information over volume alone. Hippocampi were manually outlined in MRI and volumes measured. 3D meshes were generated from the hippocampal outlines using the SPHARM toolbox and rigidly aligned. The mean meshes of the left and right hippocampi for all subjects were calculated and the distances from points on the surface of each of the hippocampal meshes to the mean meshes were calculated for each side. Linear regression analysis with correction for multiple comparisons was performed with distance from the mean surface as the dependent variable and diagnostic group as the independent variable. The first analysis was adjusted for the age, gender and total-intracranial volume. The second analysis was adjusted for age, gender and hippocampal volume to understand whether any shape differences could be found over and above volume differences. Any resultant significant areas from these second analyses would indicate that shape metrics may improve subject group separation over volume alone and therefore demonstrate diagnostic utility. Mean hippocampal volume differed by group with PCA intermediate to tAD and controls (see Table 1.). In the left hippocampus significant differences in shape were also found between PCA and control subjects (Figure 1) and between PCA and tAD subjects (Figure 2). Shape differences were more extensive when adjusting for age gender and head size; however areas of shape difference remained following adjustment for age, gender and hippocampal volume. Significant hippocampal shape differences were found between diagnostic groups.
