JOHN VISINTINE, STACEY SCHEIB, GENNADY MIROSHNICHENKO, CHRISTOPHER HARVEY, KEITH RYCHLAK, VINCENZO BERGHELLA, Thomas Jefferson University, Philadelphia, Pennsylvania, The Lankenau Hospital, Obstetric and Gynecology, Wynnewood, Pennsylvania OBJECTIVE: This study is intended to determine if the cerclage height is associated with spontaneous preterm birth (PTB) in patients with an ultrasound indicated cerclage. STUDY DESIGN: We performed a retrospective cohort study of women who received an ultrasound indicated cerclage (UC). We defined UC as a cerclage placed for a short CL ( 25mm), detected by transvaginal ultrasound between 14-23 6/7 weeks in asymptomatic women. Our data source was the Thomas Jefferson University Prematurity Database, and the Main Line Hospitals= cerclage database. Patients with multiple gestations, indicated PTB, history indicated cerclage, manual exam indicated cerclage, transabdominal cerclage and major fetal anomalies were excluded. Ultrasound measurements obtained prior to cerclage placement and the first ultrasound after cerclage placement, ( 24 week) were used for data analysis. The following measurements were obtained: functional CL (closed portion of the endocervical canal), and the cerclage height (the distance from cerclage to the external cervical os). Patients were categorized by cerclage height: 10 mm, 10-19 mm, and 20 mm. Our primary outcome was spontaneous PTB 35 weeks. Chisquare analysis was performed for categorical variables. ANOVA was performed for continuous variables. Type I error was set at 0.05 (2sided). All statistical analysis was performed using SPSS version 13, Chicago Il. RESULTS: After exclusions a total of 70 women were included in the analysis. The baseline characteristics were similar between cerclage height groups according to age, race, history of PTB, gestational age and CL time of cerclage. See table for spontaneous PTB rates according to cerclage height. CONCLUSION: Cerclage height of 20 mm is associated with a reduction in spontaneous PTB for women with a UC." @default.
