- W2124779285 abstract "Background: Colonoscopy is currently considered the best method for colorectal cancer screening. However, lesions may be missed, especially on the proximal aspect of haustral folds and flexures or behind the ileocecal valve. The Third Eye™ Retroscope™ (TER) is a new device that provides a supplemental, retrograde view during colonoscopy. We present initial results based on the first 100 subjects enrolled in a prospective study to evaluate efficacy of the device for detecting polyps that are missed during colonoscopy. Method: This multi-center study involves 12 experienced endoscopists at eight U.S. sites. Investigators have examined 100 patients (age 55-80) during screening or surveillance colonoscopy using a Third Eye Retroscope (Avantis Medical Systems, Inc., Sunnyvale, CA) in combination with a standard colonoscope (CF-Q160AL or CF-Q180AL, Olympus America Inc., Center Valley, PA). After cecal intubation, the disposable TER is inserted through the instrument channel of the colonoscope. As it emerges from the channel, the distal tip of the device turns 180° to provide a retrograde view. During withdrawal, the two video images are observed side-by-side on a wide-screen monitor. For each polyp, the endoscopist indicates whether it is visible through the colonoscope or is detected with the colonoscope only after first being detected with the TER. Results: In these first 100 individuals, 102 polyps were identified with the standard colonoscope. With the TER, 18 additional polyps were detected, a 17.6% increase (see table). With the colonoscope, 57 adenomas were found (size range: 0.1-2.0 cm; mean: 0.49 cm), and with the TER, 7 more adenomas were detected (range 0.3-0.8 cm; mean: 0.47 cm), a 12.3% increase. With the colonoscope, 45 non-adenomatous polyps were found (range: 0.1-0.8 cm; mean: 0.36 cm), and with the TER, 11 more were detected (range: 0.2-0.8 cm; mean: 0.36 cm), a 24.4% increase. None of the polyps showed advanced histology. In 14 individuals (14%), at least one additional polyp was found, and in 6 patients (6%), the polyp detected with the TER was the only one found. Every polyp that was initially detected with the TER was then located and removed with the colonoscope. There were no device-related adverse events. Conclusions: Initial results of a prospective multi-center study indicate that a retrograde-viewing device revealed areas that are hidden from the forward-viewing colonoscope and allowed detection of 17.6% additional polyps.
