- W2128049250 abstract "Dear Sir: Ultrasound-guided percutaneous ethanol treatment of hepatic rumors was introduced I as an alternative to surgical resection for rumors less than 50 mm in their greatest diameter2 We have already successfully treated rumors as large as 70 mm in diameter with very large volumes of absolute alcohol, bur repeated weekly sessions were required.' A stereotaxic approach was decided on to increase the induced necrosis and to reduce the diffusion of alcohol outside the tumor. Five patients (5 men and 1 woman; mean age, 70 years) were treated. They presented with hepatic metastases of colonic cancer in 4 cases and hepatocellular carcinoma complicating liver cirrhosis in 2 cases. The rumor measured between 6 and 8 em in diameter (mean, 7.4). Under general anesthesia, 4-6 22-gauge needles were inserted percuraneously, in a single session, in several plans at different sites inside the neoplasm. Absolute sterile alcohol (40-100 mL; mean, 68 mL) combined with Lipiodol Ultrafluide (4-5 mL; Guerbet Labs, France) was injected by the different needles under sonographic real-time conttol. When droplets of alcohol could be seen in the hepatic vessels or coming backward the needle, another needle was used for injection. The patient was discharged the following day in good clinical condition. No complication was directly related to the procedure. The clinical starus was better for the 5 patients who were followed up. One parient was lost to follow-up. Tumor marker levels significantly decreased in the 4 cases with high initial levels. Decrease in size of the imaging abnormalities on compurerized tomography and ultrasonography was observed in all cases. Pulmonary metastases were detected in I patient with colonic cancer and ascites in I parient with cirrhosis. No guided cytology was performed in these 2 patients. For the remaining 3, no malignant cell was detected wirh 3 guided cytologies performed at 1 and 3 months. These preliminary data suggesr that a large volume of alcohol, using several needles in a single session to perform a stereotaxic approach of the rumor, may be useful to reduce the volume of hepatic neoplasms." @default.
