- W2128052251 abstract "Project Description: Engagement of people with dementia is thought to be the critical element for successful psychosocial interventions. Observational measures have been developed to study engagement in residential care. However these may not be suitable in home care where the risk of a hawthorn effect is increased and travel and logistics of observations are prohibitive. The aims of the current study were to develop and validate measures of client engagement for the home care setting. The Homecare Measure of Engagement-Staff report (HoME-S) consisted of five Likert-style questionnaire items designed to specifically correspond to each of five previously conceptualised dimensions of engagement: client acceptance, attention, attitude, appropriateness, and duration of engagement. A sixth item assessed passivity. The Homecare Measure of Engagement-Client report (HoME-C) was a researcher-rated semi-structured interview designed to obtain client perspectives regarding two subscales of conversational engagement and recreational engagement during careworker visits. Careworkers (n=85) completed the HoME-S and the Working Alliance Inventory (WAI)-Bond subscale. Aged homecare clients (n=163) and/or their family (n=105) completed the HoME-C, Neuropsychiatric Inventory - agitation, dsyphoria, and apathy subscales; Positive and Negative Affect Schedule - attentiveness subscale; WAI-Bond subscale; Global Deterioration Scale; and the Cohen-Mansfield Agitation Inventory. The HoME-S and both conversational and recreational subscales of the HoME-C demonstrated good test-retest and inter-rater reliability (.85 and .78/.98, respectively). Construct validity was evidenced by significant negative correlations with apathy (r s= -.23 to -.38, p s <0.01). Lower apathy was the only independent client characteristic associated with higher conversational engagement on the HoME-C (beta= -.34, p = 0.01). There were no independent client correlates of the HoME-S or recreational engagement on the HoME-C. After controlling for client variables a stronger relationship bond between careworker and client predicted higher HoME-S scores and recreational engagement on the HoME-C (beta= .30, p = 0.02, and beta= .34, p = 0.01, respectively). The HoME-S and HoME-C are reliable and valid measures of engagement for use in home care. Home care clients who are high in apathy may be particularly difficult to engage. Care providers may be able to increase engagement by strengthening the relationship between careworkers and clients." @default.
