- W2129112684 abstract "Environmental and human factors contribute to the distribution and incidence of malaria in Latin America. They have prevented eradication of malaria and limited the effectiveness of malaria control measures. Malaria problems are greatest in Central America and the Amazon basin and least in the Southern Cone countries. Environmental factors that benefit malaria transmission and vector-breeding in many areas of Latin America are high rainfall high temperatures low altitude and remote and inaccessible areas. Socioeconomic and political factors include population movements political instability and guerilla warfare. Environment modifications to prevent vector-breeding are responsible for the elimination of malaria in Chile in the early 1940s. More than 50% of all malaria cases are in the Amazon region. Agreements and arrangements are needed to deal with movements of people within and between countries since it is difficult to control the reservoir of infection and transmission. For example the Southern Cone Pact approves the exchange of information on malaria and resources for its control. Inter-country coordination efforts are limited in much of Central America. However Costa Rica and Nicaragua cooperate on the control of malaria and dengue fever as well as on primary health care. Political instability has launched the movements of refugees. It curtails economic performance which in turn reduces the availability of resources to improve public health. Political instability also limits the initiation and maintenance of control measures in those instances in which resources are available. Malaria control programs pay little attention to the socioeconomic contexts in which they have to be applied. Malaria control programs and public health promotion programs have not adequately tried to involve communities yet community participation is needed for primary health care. The population needs education so they can better understand their health problems and the ways they can address these problems.
