Many newtechniques, such as arthroscopically assisted mini-open repair and direct arthroscopic repair, have been developed. However, on larger rotator cuff tears the standard open technique continues to be the gold standard for repair. Often, during training, the surgeon learns only the mechanical steps involved in performing the repair and does not learn the rationale behind the steps leading to a successful outcome. This article will first address the evaluation of the patient with a suspected rotator cuff tear. This will be followed by a discussion of the theory behind the choices of bone fixation, tendon preparation, and suture technique. The choice of an appropriate suture material will also be addressed. Finally, a detailed description of the surgical approach and technique used by the senior authors will be given." @default.
