- W2129333746 abstract "This study seeks to examine the immunologic response to acute psychological stress in persons with chronic pain. Chronic pain and inflammation are thought to bi-directionally influence each other but little is known about the involvement of cognition in this process. Additionally, sleep is known to have important homeostatic functions related to mood regulation, response to stress, immunity, and pain. Therefore, an understanding of the interplay between cognition, pain, and sleep in the context of the immunologic response could help to redefine cognition and/or sleep as active components of biological pathogenesis. Baseline blood pressure, heart rate, pain ratings, sleep variables, and blood samples were obtained followed by a 10-minute negative cognitive focus related to the exacerbation of the patient’s pain condition. Heart rate, blood pressure and pain ratings were again measured followed by 4 more blood draws over the course of 2.5 hours. Blood samples were analyzed for serum cortisol, IL-1, TNF-α and IL-6. Preliminary analysis of the first 17 subjects, of which only 12 responded to the stressor (N = 12), showed significant correlations between sleep, depression and a pre-existing tendency to catastrophize as well as a significant association with cortisol reactivity. The association between sleep and blunted cortisol reactivity is consistent with the results of previous studies. Trends of association with overall cortisol and IL-6 levels/ reactivity were also observed. These preliminary data suggests a direct and/or modifying role of sleep in the immunologic response to negative cognition in this sample. Data results for the full sample (N = 46) will help elucidate the significance of relationships found in this preliminary analysis. High co-morbidity between chronic pain conditions and sleep disturbance warrants inclusion of this variable in subsequent study designs and/or pain therapy." @default.
