Current studies indicate that the disease occurs commonly and significantly across all age ranges, and in the elderly in particular, where it is frequently underdiagnosed.AimTo assess the clinical, immunological, histological and developmental features of autoimmune hepatitis in elderly Tunisian patients and contrast the findings with those relating to younger adults.MethodWe reviewed retrospectively, in a multicenter Tunisian study, features of 20 patients with autoimmune hepatitis diagnosed at age ≥ 65 years and compared them with those of 63 younger patients.ResultsThe clinical presentation was similar in the elderly when compared with the younger patients, except for a higher incidence of ascites. The patients ≥ 65 years presented with a higher frequency of cirrhosis and oesophageal varices than the younger patients. However, no difference was observed in the biochemical, immunological and histological expressions of the liver disease. No significant differences were observed between the two groups regarding the frequency of concurrent immune diseases. Complete remission in treated patients was also achieved to a similar extent in both groups.ConclusionAutoimmune hepatitis is common in individuals ≥ 65 years. Elderly Tunisian patients have a higher frequency of cirrhosis at presentation than younger patients. Elderly Tunisian patients enter remission during treatment and relapse as commonly as younger patients. Autoimmune hepatitis should be considered in all elderly patients with chronic liver disease and the diagnosis warrants treatment with conventional regimens." @default.
