Therein lies the importance to clarify the pathogenesis of HCC and develop a new therapeutic strategy. The article by Schmitt and colleagues in this issue, entitled ‘Disruption of hepatocellular tight junctions by vascular endothelial growth factor (VEGF): a novel mechanism for tumor invasion’ is therefore, very interesting and may offer insights into the understanding of the mechanism for the invasive phenotype of hepatocellular carcinoma (HCC) [1]. VEGF, a potent endothelial specific growth factor, was first discovered in the bovine pituitary follicular stellate cells by Ferrara at 1987 [2]. Since then, many studies have reported on the basic and clinical viewpoints and application for angiogenic and antiangiogenic therapy. In 1983, prior to this discovery, Senger identified a molecule from the cancer cells of a mouse that increased the permeability of the vessels, the molecule became known as vascular permeability factor, VPF [2]. In 1989, the structures of VEGF and VPF were determined, and surprisingly, it was found that the two molecules were encoded by the same gene [4]. Molecular analysis revealed that the different biologically active substances were the same on the molecular basis. The role of VEGF in the development of HCC has been reported mainly from the perspective of its angiogenic potential. As a typical hypervascular tumor, HCC produces and secretes VEGF, thereby forming new tumor vessels, which provide oxygen and nutrients to cancer cells causing them to grow. In the initial phase of the HCC carcinogenesis and subsequent growth, acquirement of the hypervascular characteristic seems to be an almost essential phenotype.
