- W2129805983 abstract "Background: In our heart failure unit front-line nurses identified poor understanding of key patient outcomes among nursing care assistants (NCAs), resulting in under-reporting of changes in patient status. We recognized the need to better integrate NCAs into patient care processes. Our goal was to improve NCAs ability to understand the challenges of our patients, recognize changes in patient status, and report concerns to the nurse. A secondary purpose was to enhance staff collaboration and teambuilding. Methods: Understanding of the rationale for NCA tasks, such as vital signs, accurate intake and output, skin observations, and reporting changes to the registered nurse, are essential for early identification of changes that allow for improved patient care quality. Front-line nurses and the manager met to discuss issues, review appropriate strategies, and develop material to capture primary educational needs of NCAs. Tools were developed, including a review of the cardiac patient, skin integrity, oral hygiene, nutrition (such as monitoring sodium intake), and accuracy in measurement of intake and output. Nurses were encouraged to emphasize the importance of early identification and communication among caregivers. A simulation lab format was developed in which the NCAs were “patients”. To appreciate patient difficulties in carrying out ADL's in the context of common cardiovascular symptoms, weights were attached to ankles and abdomen were wrapped in plastic (simulating fluid overload and weight gain), a light smear of Vaseline was placed on glasses (simulating macular degeneration), and fingers were taped (simulating eating difficulties with arthritis). A pre and post evaluation was completed to determine effectiveness of the program. Results: Participants in the workshop were more confident in recognizing changes in the patient's status and reporting concerns to the registered nurse. NCAs also reported increased confidence to ask questions. With the improved comfort level, conversations between the nurses and NCAs regarding key components of care have increased by approximately 40%. Conclusion: Additional workshops have been requested and replicated by other units, and are now offered quarterly. We have recognized not only an eagerness to learn on the part of the NCAs but also a willingness of RNs to teach and integrate NCAs more fully into the patient care processes, resulting in improved patient outcomes.
