• W2130134218 abstract "Purpose: The study was undertaken to determine compliance with and the effects of a functional six week circuit type exercise programme on fitness, strength and function in an ambulant adolescent with spastic diplegic cerebral palsy. Relevance: Although cerebral palsy is a non-progressive disorder, changes that occur as a child grows lead to balance deficits, increase in energy expenditure and decrease in cardiorespiratory fitness and strength, all of which may lead to functional impairment. During adolescence these changes intensify with increase in body weight and height, compounded by adolescent inactivity. Subject: The subject was an ambulant adolescent with cerebral palsy in this single subject design study. Method: A functional circuit type exercise programme was undertaken for six weeks, with daily activity levels recorded during this time and for the subsequent six weeks to gauge compliance with the programme and to measure physical activity undertaken. Outcomes measured before and after the exercise programme and again after a further six weeks were strength of upper and lower limb muscle groups using dynamometry, the six-minute walk test, Energy Expenditure Index (EEI), maximal oxygen consumption (VO 2 max) and the Gross Motor Function Measure (GMFM). Results: Strength improvements were evident in the lower and upper limb muscle groups that were preferentially exercised. The subject reached VO 2 max at each measurement session. The GMFM, six-minute walk test and EEI showed small improvements. A change in lifestyle activity levels was evident over the course of the study. Conclusion: A functional circuit type exercise programme may lead to improved fitness, strength and function in ambulant adolescents with cerebral palsy.
