- W2131184941 abstract "In areas of intense malaria transmission individuals can receive hundreds or thousands of infective bites from mosquitoes. Not all infections cause severe disease but disease severity is increased when infected erythrocytes (IEs) bind host endothelium and sequester in specific microvasculature such as the brain and placenta. Protective immunity to severe disease develops during the first years of life and thus malaria is primarily a childhood disease. Pregnant women are an exception and are susceptible to Plasmodium falciparum infections particularly during first pregnancies (primigravidas). The clinical manifestations of malaria that occurs during pregnancy are influenced by malaria endemicity and the pregnant woman’s immune status. When the pregnant woman has low levels of malarial immunity both mother and fetus can experience severe disease and fatal outcome including severe maternal anemia and stillbirth or abortion. Women in high-transmission regions generally have greater preimmunity and less severe malarial complications but their fetuses are at greater risk for premature delivery and low birth weight which is an important risk factor for neonatal mortality.
