- W2133661036 abstract "An 80-year-old woman had exfoliation material at the papillary area and on the surface of the anterior lens capsule in the right eye. Intraocular pressure (IOP) was well controlled with medication. Mild cataract without significant phacodonesis was noted. Phacoemulsification and aspiration (PEA) and intraocular lens (IOL) implantation with trabeculotomy were performed. During continuous curvilinear capsulotomy (CCC), phacodonesis of the mobile lens was found along with anterior chamber shallowing. After a 4–5-mm diameter CCC was performed, PEA was carried out carefully. During PEA, a zonulolysis without vitreous prolapse was found between the 7 and 9 o'clock meridians (60°). An acrylic IOL (AcrySof, MA60BM, Alcon) was implanted in the capsular bag. Thirty-six days later, the patient complained of decreased visual acuity. The anterior capsule was completely closed and the IOL was dislocated (Fig. 1). IOL extraction with capsule, anterior vitrectomy and sulcus suturing of a posterior chamber IOL were performed. We examined the extracted IOL with a scanning electron microscope. Complete closure of the anterior capsulorrhexis was observed. The region of the capsule where there was zonular attachment was shrunken and dislocated towards the centre (Fig. 2A). Fibroblast-like cells were found in the area of opacity at the central region of the anterior capsule (Fig. 2B). To prevent such an anterior capsule contraction syndrome (Masket 1993), Nd : YAG laser anterior capsulotomy is required (Davison 1993) as soon as anterior capsule contraction is found. When severe contraction occurs, removal of the IOL, anterior vitrectomy and IOL suturing are needed. If risk factors such as exfoliation syndrome (Davison 1993), diabetes mellitus (Hayashi et al. 1998a), uveitis (Davison 1993), retinitis pigmentosa (Hayashi et al. 1998b) and zonulolysis during surgery are present, careful follow-up is necessary to prevent possible anterior capsule contraction.
