- W2133896122 abstract "RATIONALE: Oral food challenges (OFC) remain the gold standard for food allergy diagnosis, but carry significant risk for the patients. We examined the significance of combining skin prick testing (SPT), specific IgE level, history of asthma and initial symptom severity in predicting a systemic reaction (other than skin) after OFC. METHODS: Retrospective review of 536 OFC to milk, egg and peanut over a 3.5 year period. RESULTS: 45% of OFC were positive with no significant difference among tested foods. There were 30% cutaneous, 31% gastrointestinal and 39% upper/lower respiratory reactions. A statistical significant increase of systemic reaction after OFC was found in children with history of: a) initial systemic reaction to tested food (34%vs 51% p<0.0001); b) asthma (65%vs52, p<0.009); c) higher levels of specific IgE (±SE) for tested food (9.2±1.2 vs 2.8±.52, p<0.00001); d) bigger wheal(±SE) (mm) at SPT for tested food (8.1±.23 vs 4.9 ±.19 p<0.00001); e) bigger flare (mm) (±SE) at SPT (20.3083± .4886238 vs 14.2±.50, p<0.00001). Using logistic regression, all parameters besides asthma, were statistically significant and associated with systemic reactions. Assigning score of 1 for: food specific IgE >5kU/L, wheal >5mm, flare>5mm, history of asthma, history of systemic reaction, a score= or >3 was significantly associated with systemic reaction after OFC (71% vs 46%, p<0,0001). CONCLUSIONS: These preliminary data could lead to development of a simple scoring system, that would help to individuate subjects with higher risk of severe reaction after OFC's." @default.
