Women with the Finnish type inactivating mutation of the FSH receptor have variable, but usually normal, development of secondary sex characteristics, although their estradiol (E2) and gonadotropin concentrations are equal to those seen in postmenopausal women (2). We treated these patients with high doses of recombinant FSH and hCG and observed no effect on serum E2, testosterone (T), or inhibin concentrations (1). Therefore, peripheral aromatization of androgens would seem to be a possible explanation for the normal breast development. Although the affected women have functional LH receptors, the treatment with hCG failed to raise the serum T concentration. It is possible that the high circulating LH concentration had already induced theca-cell androgen production to the maximum. Similarly, in a recent study on a woman with FSH deficiency, administration of high doses of hCG had only limited effect on serum androgen concentration, whereas priming of the ovaries with FSH before hCG treatment resulted in higher androgen production (3). The normal or pronounced circulating androgen concentrations in subjects with an inactivating FSH receptor mutation may also be due to a relative increase of the ovarian stroma in comparison to the follicular cells. Furthermore, the follicular conversion of androgens to estrogens is also likely to be impaired in the affected women. In addition, compensatory adrenal androgen production cannot be excluded. Although we cannot exclude the possibility that response to recombinant LH may be different from that to hCG in these women, we think that the above-mentioned theories for significant androgen production and peripheral estrogen conversion offer more plausible explanation. Unfortunately, further experiments on these patients with no clear clinical gain are ethically problematic." @default.
