- W2136393485 abstract "Many people with chronic pain report psychological distress, namely anxiety and depression. Age differences in these symptoms remain unclear. This may be due, in part, to age differences in the psychometric properties of scales designed to measure anxiety and depression. The objectives were to: (i) to compare the psychometric properties of the Hospital Anxiety and Depression Scale (HADS) in younger and older people with chronic pain. (ii) to evaluate the relationship between HADS scores and measures of pain intensity and quality. Elderly (n=159, mean age=70..9 yrs ± 8.4) and younger (n=183, mean age 41.6 yrs ± 9.3) patients with chronic pain seen at the McGill Montreal General Hospital Chronic Pain Centre completed the HADS, McGill Pain Questionnaire (MPQ) and numeric ratings (NRS; 0-10) of usual, least and worst pain intensity. Internal consistency (Cronbach's Alpha) of the Anxiety (Elderly: α = .84; Younger: α= .80) and Depression (Elderly: α= .79; Younger: α=.84) subscales of the HADS were comparable in younger and older patients. HADS total and subscale scores were not significantly different between the groups. The percentage of patients in each age group scoring above the cutoff score for depression was not different (Young: 62.0%; Elderly: 56.5%). However, a significantly higher percentage of younger (72.8%) than older (59.7%) people scored above the cutoff for anxiety (pH .02). There were age differences in the pattern of correlations between the HADS, MPQ and NRS. The HADS has comparable psychometrics across the age groups. However, the prevalence of anxiety is higher in younger than older chronic pain patients. There is some evidence that the relationship between HADS scores and measures of pain intensity and quality also differ between the age groups. The implications of these findings for the assessment of pain and psychological distress will be discussed Many people with chronic pain report psychological distress, namely anxiety and depression.
