- W2138372232 abstract "To report our initial clinical experience of radio-embolization, in which patients with liver metastasis were treated with intrahepatic Yttrium-90 microsphere through the hepatic arterial circulation. Data from 20 SIR-Spheres® Yttrium-90 microsphere radio-embolization procedures were reviewed. Patients received no antineoplastic therapy 4 weeks prior to or following the procedure. Ages ranged from 35 to 87 years. Pre-treatment hepatic angiogram confirmed anatomy, and embolization of the necessary arteries was performed. The primary tumor was neuroendocrine in 50% and other primary sites in 50%. Pretreatment scans were performed to ensure no excess lung shunting. The entire liver was treated in 75%, the right or left lobe selectively in 25%. Volume of liver treated ranged from 223–2630 (mean, 1378 cc). The range of dose delivered was 0.56–1.61 GBq. Radiographic response was evaluated with CT approximately 4 weeks following treatment. Follow-up ranged from 5–10 months. The treated dose was within 11% of the intended prescribed dose in all but one patient who developed static flow and received 41% of the prescribed dose. In all but a single patient, arterial flow slowed but did not reach stasis. The majority of patients had minimal side effects. The CTC V 3.0 grade 1 adverse events included 35% fatigue, 25% anorexia, 25% nausea, 20% abdominal pain and 10% vomiting. One patient had symptoms which lasted more than 1 week following treatment. Of the evaluable patients, radiographic tumor response has been seen in 71% at 4 weeks following treatment. Two patients have shown progression of disease and both have died within 3 months of treatment. Radiographic response has consisted of lesion necrosis, decrease in vascularity and decrease attenuation of the lesions. Those patients with radiographic response have also been found to have normal or stabilized liver function tests following treatment. No new liver lesions were identified in patients with radiographic response (Figure). Our initial clinical experience with Yttrium-90 radio-embolization for selected patients with liver metastasis has been encouraging. Radiographic response has been seen in 71% of patients with acceptable acute toxicity. Further evaluation with longer follow-up will help clarify the benefit of this novel treatment." @default.
