- W2140097910 abstract "While birth defects have been associated with prematurity and low birthweight, numerous confounding factors have limited the ability to draw conclusions about the independence of this risk factor. Propensity scoring, a methodology used to address other complex medical outcomes, was applied as a novel approach to address the independent effect of birth defects (chromosomal and congenital structural abnormalities) on prematurity and low birthweight. A large prospective multicenter database (The FASTER Trial) was studied. All livebirths were analyzed according to two groups: (1) chromosomal or structural abnormality and (2) normal. Propensity scoring allowed for multidimensional balancing on over 20 potential confounders such as BMI and smoking to minimize bias in the assessment of the effect of abnormal status. The effect of abnormal status on: 1) preterm birth (<37 weeks gestation, includes both spontaneous and indicated), 2) low birthweight (<2500 g), and (3) very low birthweight (<1500 g), was assessed using multiple logistic regression analyses adjusting for the propensity scores and other important confounders. The study included 33,073 singleton livebirths. Liveborns with a birth defect were 4.8 times (95%CI: 4.4-5.4) more likely to deliver preterm. Babies with birth defects were 5.1 (4.5-5.7) and 22.4 (16.2-31.1) times more likely to be low birthweight or very low birthweight, respectively, after adjusting for the propensity scores, maternal age, diabetic status prior to pregnancy, preeclampsia, placental abruption, placenta previa, IUGR, and other pregnancy complications. Birth defects are independently associated with prematurity, low birthweight, and very low birthweight, when controlling for multiple confounding factors, including shared risk factors and pregnancy complications, using propensity scoring and regression analysis.
