Few studies have been reported about the relationship between the expression of neurotrophic factors (NGF) and detrusor hyperactivity. Although it is reported that α-1 adrenoceptor (α1-AR) antagonist and PDE5i improves not only voiding symptoms but also storage symptoms, the mechanism remains controversial. This study is designed to investigate the effects of tamsulosin and sildenafil on central micturition reflex by the degree of neuronal activation and NGF expression in the central micturition areas. METHODS SHR and Wistar-ST (Wistar) rats were used for experimental animals. Rats were randomly divided eight groups (n = 8, in each group): the Wistar-control group (G-A), the SHR-control group (G-B), the SHR and 0.01mg/kg, 0.1mg/kg, and 1mg/kg tamsulosin-treated group (G-C to G-E), the SHR and 1mg/kg, 5mg/kg, and 10mg/kg sildenafil-treated group (G-F to G-H). Tamsulosin or sildenafil-treated groups received orally once a day for 30 days. Immunohistochemistry for c-Fos and NGF were performed for the detection of neuronal activity in the medial preoptic area, ventrolateral periaqueductal gray, pontine micturition center and spinal cord; L4-L5. Parameters of awake urodynamics were investigated before and after treatment in Wistar and SHR. The mean blood pressures (BP) of the SHR and Wistar were compared using polyethylene catheter inserted in the femoral artery and connected to a pressure transducer (Harvard Apparatus). RESULTS The mean BP in the SHR increased compared with Wistar. The SHR (G-B) showed enhanced non-voiding contraction (NVC) pressure and duration in compared to control (G-A) . In treatment groups (G-C to G-H), it was significantly suppressed. This suppression was reversely proportional to the dose of tamsulosin and sildenafil. The expression of c-Fos and NGF in all the central nerve system (CNS) regions was significantly enhanced in G-B compared to G-A. In treatment groups (G-C to G-H), it was significantly suppressed in all the CNS regions. This suppression was also reversely proportional to the dose of tamsulosin and sildenafil as the pattern of NVC.
