Coronary heart disease is currently divided into Chronic Coronary Artery Disease (silent myocardial ischaemia, stable angina pectoris, ischaemic cardiomyopathy) and Acute Coronary Syndrome (unstable angina pectoris, NSTEMI = non-ST-elevation myocardial infarction, STEMI = ST-elevation myocardial infarction). Although beta- blockers have been shown in prospective randomized clinical trials to reduce morbidity and mortality only fol- lowing myocardial infarction, all recent guidelines from leading international societies (European Society of Cardio- logy, American College of Cardiology, American Heart Association) recommend beta-blockers to be given a class- 1 classification in all forms of coronary artery disease. On the other hand, data from major clinical trials in patients with myocardial infarction are overwhelming showing a re- duction of morbidity and mortality ranging between -23 and -64 %. In addition, it has been found that patients with suspected contra-indications to beta-blockers (older age, presence of heart failure or chronic pulmonary obstructive disease) may benefit rather more than less from beta- blockers following myocardial infarction. Therefore, at this time and age, withdrawal rather than administration of beta-blockers to patients with myocardial infarction needs special reasons. J Kardiol 2003; 10 (Suppl A): 10-3." @default.
