- W2144821212 abstract "To determine whether the combined analysis of volumetric and metabolic information by F18-FDG PET/CT, i.e. summation of the standardized uptake values (SUVsum) in all the tumor tissues, could provide a better evaluation of prognosis for non-small-cell lung cancer (NSCLC). We identified 105 consecutive NSCLC patients who received staging PET/CT scans before any therapy between April 2005 and September 2007. These patients were free of brain metastasis within one month after PET/CT, received treatment, and had subsequent clinical follow-up in our hospital. Total metabolic tumor volume (MTV) and average standardized uptake value (SUV) of the tumor were determined on pretreatment PET scans by commercial software. SUVsum is the summation of individual tumor volume multiplied by its SUV. The relationship between SUVsum, MTV, maximum SUV (SUMmax), progression free survival (PFS), and overall survival (OS) were analyzed. Univariate and multivariate analyses were performed to assess the prognostic significance of SUVsum and other factors. The median follow-up time for the living patients was 3.1 years. There were 28 patients with stage I, 8 patients with stage II, 46 patients with stage III and 23 patients with stage IV disease. The estimated median progression-free survival (PFS) and overall survival (OS) for the cohort were 10.8 months and 2.8 years. The SUVsum ranged from 1.53 to 3289.76 with a median of 143.99. The SUVsum and MTV were significantly and inversely correlated with disease progression (r = -0.001; p < 0.001 for SUVsum; r = -0.009; p < 0.001 for MTV) in univariate analysis. On multivariate Cox proportional hazards model, SUVsum (p < 0.001), but not MTV (p = 0.51) and SUMmax (p = 0.76), remained significant in PFS after controlling for stage, treatment, age, sex and pathology. For stage III NSCLC patients, a cutoff of 650 for the SUVsum was a significant prognostic factor for PFS (p = 0.001). On multivariate analysis, high SUVsum (> 650) also showed a significantly poorer PFS than those with low SUVsum (< 650) in stage III NSCLC (p = 0.01). High tumor burden assessed by F18-FDG PET SUVsum is an independent poor prognostic factor for NSCLC. It may be a promising tool for stratifying stage III patients for risk-adapted therapies." @default.
