- W2148551176 abstract "Background: Congestive heart failure (CHF) is characterized by neurohumoral activation, sodium retention, ventricular dysfunction and congestion, and evolves from asymptomatic, to stable symptomatic to overt symptomatic CHF. The natriuretic peptide system (NPS), including atrial (ANP) and brain (BNP) natriuretic peptides is progressively activated during progression of CHF. Levels of NPS components have been found useful in the diagnosis and prognosis of CHF in various populations. Evaluation of NPS components as biomarkers for patient markers is a high priority as little is known about the degree of variability of ANP, BNP, and their N-terminal forms, NT-proANP and NT-proBNP, in normal, stable and recently hospitalized CHF subjects over time. This has implications for clinical monitoring of CHF patients with the NPS. Methods: We characterized the variability (coefficient of variation) of these natriuretic peptide forms. ANP, NT-proANP, BNP, and NT-proBNP levels were determined in normal (n=10, blood draws 10 consecutive weekdays), stable CHF (n=19, twice weekly, 4 weeks) and recently hospitalized CHF (n=20, in hospital, then twice weekly, 4 weeks) subjects. Results: We found no difference in the coefficient of variation among the NPS peptides in normals. In the stable group, NT-proANP showed less variability (21%, p<0.05) than ANP (37%) or BNP (36%) but not than NT-proBNP (26%). NT-proBNP did not vary compared to the other NPS peptides in the stable group. In the recently hospitalized group NT-proANP showed less variability (24%, p<0.05) than ANP (43%); BNP (33%) and NT-proBNP (33%) did not vary compared to other NPS peptides. There was no difference in variability of ANP, NT-proANP, BNP, or NT-proBNP when each was compared across groups. Conclusions: We report that the components of the NPS evaluated in these studies differ in variability with CHF. NT-proANP exhibits relatively minimal variation among the NPS peptides evaluated in the setting of stable and recently hospitalized CHF. Repeated measures of each of these respective peptides do not demonstrate significant variability between normal, stable or recently hospitalized CHF subjects. Further studies are warranted to define the role of NT-proANP in monitoring subjects with stable CHF and in those recently hospitalized with CHF." @default.
