- W2149113480 abstract "The paper by Weichselbaum et al.’ entitled, “Toxicity of Aggressive Multimodality Therapy Including cis-Platinum, Methotraxate With Radiation and/or Surgery for Advanced Head and Neck Cancer” represents the application of a relatively old concept that has come of age. The concept involves the use of systemic treatment in the form of chemotherapy prior to localized treatment with radiotherapy and/or surgery. The purpose of the prior chemotherapy is to render the tumor more susceptible to definitive treatment. In the case of head and neck cancer, reduction in size of the primary (upstaging, if you will) provides the potential for delivering definitive radiotherapy and/or surgery to a larger proportion of patients. It is no secret that surgery, radiotherapy, and chemotherapy have often played independent, and in many instances poorly defined and competitive roles in the treatment of certain forms of cancer. It is essential, indeed critical, to the success of the multidisciplinary research, such as that reported by Weichselbaum et al..’ that there be close integration and interaction between the various treatment disciplines. objective tumor regression in close to 90% of patients, with complete clinical regression occurring in some 30% of patients.’ Thus, a relatively common squamous cell carcinoma has become one of the most chemosensitive of tumors. This of itself is of extraordinary interest. It means that the multi-modality head and neck cancer team, which includes a head and neck cancer surgeon, radiation oncologist and medical oncologist, can proceed with neoadjuvant or induction chemotherapy with little risk that the tumor will progress and a high probability that substantial tumor regression will occur." @default.
