- W2149173192 abstract "Renal dysfunction is a well-known prognostic marker in patients (pts) with acute heart failure (AHF). Generally, glomerular filtration rate (GFR) is used for assessing renal dysfunction. A new equation to estimate GFR from the Berlin Initiative Study (BIS) has been recently introduced to be more accurate than Modification of Diet in Renal Disease (MDRD) and the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation in persons aged 70 years or older. We hypothesized that estimation of GFR by BIS1 equation would have a better prognostic power in predicting cardiovascular (CV) events than MDRD and CKD-EPI equation in elderly pts with AHF. We retrospectively analyzed data from the Korean acute Heart Failure (KorHF) Registry which enrolled hospitalized pts with treatment for AHF (n=1413, 563 males, 78 ± 6 years old, 588 ischemic origin, left ventricular ejection fraction 40.8 ± 15.8 %). The CV events were composite of all-cause mortality and HF rehospitalization. Mean GFR by MDRD, CKD-EPI and BIS1 were 56.0 ± 29.5, 49.9 ± 21.6 and 45.7 ± 16.5 ml/min/1.73m2. The prevalence of renal dysfunction (eGFR < 60 ml/min/1.73m2) by MDRD, CKD-EPI and BIS1 were 61.8%, 67.6% and 82%, respectively. During follow-up period (median 305 days, IQR 56-743), 302 (21.4 %) pts died and 305 (21.6%) pts were rehospitalized. In multivariate Cox proportional hazard analysis, CKD stage 3b (30 < eGFR < 45 ml/min/1.73m2) only by MDRD (HR 1.461, 95% CI 1.047 - 2.040, p = 0.026) was an independent predictor of CV events after adjusting other HF risk factors, whereas neither by CKD-EPI nor BIS1. The net reclassification improvement for eGFR by BIS1 over MDRD and CKD-EPI were -4.88 and -6.68%. We found that estimated GFR by a new equation, BIS was not more predictive of CV events in elderly AHF pts than that by MDRD and CKD-EPI equation." @default.
