- W2151062645 abstract "This chapter presents a review of the present status of the knowledge concerning the mechanism of the carcinoid flush. Patients with the carcinoid syndrome often show an extreme sensitivity to alcohol and ingestion of a moderate amount may produce a deep and long-lasting flush. The major metabolic route for 5-HT in man is oxidative deamination and the resulting 5-hydroxyindoleacetaldehyde is predominantly oxidized further to 6-HIAA. Several reports have indicated that ethanol ingestion interferes with this major route of 5-HT metabolism in man. It is demonstrated that serotonin-14C metabolism in normal subjects and in patients with carcinoid tumors is markedly altered by ethanol ingestion. In patients with the carcinoid syndrome and marked overproduction of 5-HT, alcohol ingestion results in greatly increased and prolonged local levels of 5- hydroxyindoleacetaldehyde and 5-hydroxytryptophol. These studies demonstrate that ethanol ingestion diverts the metabolism of the aromatic aldehydes derived by oxidative deamination of the corresponding biogenic amines from the normal predominantly oxidative route to a reductive pathway." @default.
