• W2151085273 abstract "Project Description: The Specialist Cognitive Disorders service at the National Hospital for Neurology and Neurosurgery in London is a tertiary referral centre offering diagnosis and management, typically for patients and families with rare, inherited and early-onset dementias. In response to NICE guidelines and a growing evidence base on the effectiveness of psychosocial interventions in dementia the service recently expanded to include a clinical psychologist. The psychology service offers an assessment session, followed by short-term intervention (typically three sessions). The service aims to offer psychoeducation, support, and address the mental health needs of patients and families around the time of dementia diagnosis, predominantly using a cognitive-behavioural model. Currently 41% of referrals are for a patient with dementia, 39% for carer or family and 20% for both although in practice patients are often seen with a partner. Twenty percent have a diagnosis of sporadic AD, 24% a primary progressive aphasia (PPA), 17% posterior cortical atrophy (PCA), 12% behavioural variant fronto-temporal dementia (FTD), 7% DLB, 2% inherited (familial) Alzheimer's disease (FAD), with the remainder various (e.g. HD phenocopy, MCI, PD, PSP). 68% of these have been diagnosed prior to the age of 65 (mean (SD) 54 (8) years). Reasons for referral are: anxiety (particularly PCA); low mood (particularly AD); frustration and irritability (particularly PPA); coping with behaviours that challenge (particularly families of FTD); relationship difficulties (cross-diagnosis, particularly where couples' pre-morbid coping strategies differed from each other). Themes specific to people with rarer dementias are: lack of understanding of non-amnestic symptoms; difficulty explaining diagnosis to others; perception that local services (e.g. memory clinics) have not heard of the rare dementia and therefore cannot help adequately; services (e.g. day centres) not catering for younger people; concerns about heritability; and effect on young children. To date everyone referred to the service has attended an assessment and intervention sessions. Patients and families report better understanding of symptoms, as well as triggers and maintaining factors for anxiety, low mood and frustration. Referral rate is increasing and plans are to increase staff numbers to meet this need, as well as to develop better quantitative outcome measures for this diverse population." @default.
