- W2151558027 abstract "Background: Recently Olympus-Keymed (UK) has launched the Lucera Video Endoscopy processor with the advanced features of adaptive IHb colour and structural enhancement. Combined with a high resolution magnification endoscope, it emphasizes subtle chromatic and structural mucosal alterations, highlighting the pit patterns and the microvascular architecture. Our aim was to systematically study the Upper Gastrointestinal (UGI) tract with the above endoscopy system. Methods: We report our experience from the first 70 patients who underwent UGI endoscopy with a high resolution magnification gastroscope (Olympus, Q240Z) and Olympus Lucera CV 260 video endoscopy processor. Results: We were able to distinguish in all patients the normal gastric antral and body microvasculature. In gastric antral mucosa the microvasculature shows a regular coil-shaped subepithelial capillary network (SECN). In the gastric body mucosa, microvasculature demonstrates a honeycomb-like SECN pattern and collecting venules in a regular arrangement. In 1 patient with endoscopic appearance of a flat reddened antral area, we identified the disappearance of the regular SECN pattern, and visualised a demarcation line separating normal from abnormal capillary network. Histology showed high grade dysplasia (HGD). In 1 patient who was evaluated for chronic iron deficiency anaemia refractory to iron replacement therapy, and an extensive investigation had revealed only antral gastritis, we detected dilated telangiectatic capillaries and petechial haemorrhages as typically seen in Watermelon Stomach. The patient was treated with Argon Plasma Coagulation and remains well with no further blood loss. In 5 patients with fundic gland polyps the appearance was that of body-type mucosa while in 3 patients with adenomas there was villous pit pattern and increased vascularity. We examined the duodenal villi in all patients, and showed patchy shortening of villi in a patient with coeliac disease. In 25 patients with Barrett's oesophagus there was increased vascularity, which was especially pronounced in 2 patients. Histology confirmed low grade dysplasia in one and HGD in the other. Conclusion: High resolution magnification endoscopy with adaptive IHb enhancement can be a powerful tool for the detection of subtle GI pathology. Further experience is needed in order to estimate its true diagnostic efficacy." @default.
