- W2153244989 abstract "OBJECTIVE: Gentle ovarian stimulation protocols have been thought to produce less eggs, but eggs of high quality, and be easier on the patient. Our objective was to evaluate a very mild, low cost minimal stimulation protocol. DESIGN: Retrospective study of consecutive unselected cycles in a large American clinic. MATERIALS AND METHODS: The following protocol was used in 2,957 consecutive cycles with no pre-selection of patients: Clomiphene citrate 50 mg was initiated on Day 3 and continued until the follicles were developed sufficiently for ovulation triggering with a gonadotropin releasing hormone agonist (GnRa). Very low supplemental doses of 150 IU of FSH were begun on Day 8. Oocyte retrieval was performed under ultrasound guidance with no sedation using a 21-gauge needle. Single embryo frozen blastoyst transfers (SET) were performed under ultrasound guidance. RESULTS: A total of 1,503 SET transfers were performed. There were only 4% twins and no triplet or greater pregnancies. The success rate was higher with frozen embryo transfer cycles than with fresh embryo transfer cycles (41% versus 20%) (P<0.05). The FSH level did not have a significant effect on pregnancy rate per transfer (p> .05) although it did affect the cancellation rate and the transfer rate. For women under 35 years of age and FSH less than 15 for single embryo fresh transfers, the fresh embryo pregnancy rate was 26.8%, and the frozen transfer pregnancy rate was 47.7% (P<0.05). Even for FSH >15, the pregnancy rate for women under 35 years of age was 52.5% although in this one category, 2 embryos were transferred rather than one. CONCLUSION: Favorable pregnancy rates can be achieved inexpensively using mini-IVF protocol, and avoiding pain and complications such as OHSS. Vitrification for reliable and cheap embryo freezing facilitates patient acceptance of SET, and increases pregnancy." @default.
