- W2154736859 abstract "Objective: To estimate the prevalence of occult depression in adolescent (aged 13- 17 years) emergency department (ED) patients and to determine patient characteristics associated with depression. Methods: Medically stable adolescent patients presenting to three urban emergency departments for nonpsychiatric reasons were approached for enrollment from Sept 29, 2005 to March 1, 2007. After parental consent and patient assent, eligible patients were given the Beck Depression Inventory-II (BDI-II),and also answered questions about their attitudes, activities and life style choices. Parents were also asked similar questions and about family demographics. The BDI-II was scored at the patient’s bedside and patients and parents were told of the results. When appropriate, referrals were made for immediate psychiatric evaluation or for out- patient assessment. Group results were analyzed with descriptive statistics;patient characteristics associated with depression were determined by multivariate analysis. Results: Nine hundred sixty seven patients were enrolled. When asked prior to formal screening, 21% of enrolled patients indicated that they thought they were depressed; 33% of these were not recognized by their parents as being depressed. With formal depression screening by the BDI- II, 20%(n= 197) of patients had BDI-II scores indicating moderate to severe depression.Only 58% recognized that they were depressed and only 50 % of parents indicated that their child was depressed.When compared to nondepressed patients, depressed children were more often female, had known someone who intentionally hurt themselves, or had used street drugs. Lower socioeconomic status, family stability, or witnessing violence were not associated with a positive depression screen among these patients. Conclusions: Depression occurs frequently among adolescents and is often unsuspected. Many previously identified predictors of depression were not found among these depressed patients. Routine depression screening of adolescent ED patients might determine which adolescent patients are in need of mental health care." @default.
