- W2154957312 abstract "OBJECTIVE: Determine the effects of exogenous testosterone supplementation on ovarian folliculogenesis and steroidogenesis as recent studies suggest androgens may play a role in follicular recruitment. DESIGN: A prospective randomized double-blind placebo-controlled crossover study. MATERIALS AND METHODS: Regularly menstruating healthy women between 38-45 years of age, with 2 ovaries and a uterus, not on hormonal medications, and willing to use 2 forms of non-hormonal contraception were recruited to participate. Women with PCOS, signs of hyperandrogenism, obesity (BMI>30), smokers and those attempting to conceive were excluded from the study. Participants received a 12 day course of transdermal testosterone (Androderm 2.5mg) or placebo patch followed by 7 days of gonadotropin stimulation. Transvaginal ultrasounds and bloodwork were obtained at baseline, after treatment, and after stimulation for each patient. Following ovarian stimulation, medroxyprogesterone acetate was used to induce menses. After a one month wash-out period, the subjects underwent the same protocol using the opposite treatment. Data were analyzed using the paired t-test. RESULTS: 12 subjects were recruited with an average age of 42.3±2.7 years, average BMI of 24.6±3.8, and an average cycle length of 27.1±3.2 days. Total and free testosterone levels were significantly increased following testosterone treatment compared to placebo (312.7 vs. 12.3, and 45.5 vs. 1.4 respectively, p<0.001) while there was no significant difference in LH, FSH, or estradiol levels. Estradiol levels were also comparable after administration of gonadotropin. The total number of follicles (>2mm) recruited following gonadotropin stimulation was similar for both treatments (11.92 for placebo and 11.17 for testosterone, p=0.6), however there was a trend toward increasing numbers of larger follicles (>18mm) with the use of testosterone (0.67 vs. 0.25, p=0.096). No significant side effects were reported during testosterone administration. CONCLUSIONS: Despite significantly increased testosterone levels, a short course of androgens had no significant effect on the number of recruited follicles, estradiol levels or follicular growth following gonadotropin stimulation, although there was a trend towards higher numbers of large follicles. Further studies are needed to better determine androgen's role in ovarian folliculogenesis using longer durations of androgen exposure." @default.
