- W2155394172 abstract "In this issue, Lockey et al. from London's Air Ambulance present their concept of a common algorithm for effective management of traumatic cardiac arrest, including resuscitative thoracotomy in penetrating chest injuries. 1 Lockey D.J. Lyon R.M. Davies G.E. Development of a simple algorithm to guide the effective management of traumatic cardiac arrest. Resuscitation. 2013; 84: 738-742 Abstract Full Text Full Text PDF PubMed Scopus (82) Google Scholar According to the authors, the algorithm does not necessarily distinguish between prehospital and in-hospital care. However, their approach of delivering resuscitative thoracotomy by non-surgeons to the trauma patient in the field with cardiac arrest, instead of scoop and run, will be perceived as provocative; emergency room thoracotomy by surgeons is well established but even this is controversial. Let's face it: enormous efforts have been undertaken for decades to educate, train and perform high quality resuscitation in patients with out-of-hospital cardiac arrest even though the majority are in asystole and have dismally low survival rates. Now the London HEMS group have initiated a new and radical approach by performing standardised resuscitative thoracotomy in the field. And they have been successful – achieving survival rates of 18% in patients with cardiac arrest following penetrating chest injuries. 2 Davies G.E. Lockey D.J. Thirteen survivors of prehospital thoracotomy for penetrating trauma: a prehospital physician-performed resuscitation procedure that can yield good results. J Trauma. 2011; 70: E75-8 Crossref PubMed Scopus (134) Google Scholar Following non-traumatic cardiac arrest, worldwide survival to hospital discharge is 8–11% for all-rhythm and 21–22% for ventricular fibrillation. 3 Nolan J.P. Since 2005, published survival rates have improved reasons that are not clear. 4 Working Group Ad Hoc Subcommittee on Outcomes American College of Surgeons – Committee on Trauma Practice management guidelines for emergency department thoracotomy.
