- W2155998692 abstract "Methicillin-resistant Staphylococcus aureus (MRSA) is increasingly prevalent, and dealing with it consumes ever more healthcare resources. In many situations attempts at control have been based as much on a microbiological determination to detect carriage and transmission as on clinical needs. As infection control workers have failed to curtail spread, the need is now to rationalize and prioritize our efforts. The time has come to recognize the futility of attempting to eradicate MRSA from everybody, particularly those not suffering disease as a result of it and those who are not at risk. We should rather concentrate on those who are at high risk of serious morbidity and mortality. Thus, the orthopaedic surgeon who sees hip replacements infected with MRSA has a clinical problem and needs to identify his MRSA patients before surgery in order to minimize the risks. Similarly the intensivist who encounters MRSA pneumonia once a week, sometimes resulting in death, needs a new and more successful approach. In contrast, the problem of elderly patients in nursing homes, who may be colonized with MRSA in a pressure sore, but without serious morbidity, warrants less attention. The central issue then is to identify the population at high risk of severe morbidity and mortality from MRSA, and to deal with them on clinical rather than microbiological criteria." @default.
