- W2157316695 abstract "<h2>Abstract</h2> In a controlled prospective survey it was found that elderly bedridden patients of either sex were more likely to acquire gentamicin-resistant enterobacteria than other patients. The presence of an in-dwelling urinary catheter or a degree of compromise to the host defence system similarly predisposed to acquisition. The administration of a peri-operative prophylactic regimen or treatment with broad-spectrum antibiotics, but not flucloxacillin, increased the chances of a patient acquiring resistant enterobacteria. Treatment with ampicillin, co-trimoxazole or gentamicin all encouraged the development of carriage of such strains. Gentamicin-resistant klebsiellae but not <i>Escherichia coli</i> or <i>Citrobacter freundii</i> were involved in cross-contamination. The 260 patients who acquired gentamicin-resistant enterobacteria formed only 0·6 per cent of the hospital population. Gentamicin-resistant enterobacteria caused between 1·7 and 7·0 per cent of all urinary tract infections during the outbreak and approximately 2 per cent of all cases of bacteraemia and wound infection. Bacteraemias due to gentamicin-resistant klebsiellae were more frequently fatal than those due to less resistant strains." @default.
