This manual provides rapid reference to drug therapy of cardiac diseases including hypertension and hyperlipidemia. The 12 chapters in the book are intended to answer the question of which drug to choose. Minimal coverage of pathogenesis, physiologic characteristics, and diagnosis of cardiovascular disorders is found. Information on drugs is presented in a consistent fashion; supply and dosage are discussed first, followed by pharmacokinetics, advice on efficacy, comparison with other drugs, indications, adverse effects, and drug interactions. The book is not intended as a comprehensive textbook of pharmacology, and therefore no in-depth description of molecular basis of action of drugs is provided. The first three chapters discuss calcium antagonists, beta-adrenoceptor blockers, and diuretics. The commonly used drugs are compared and contrasted in table form, thus facilitating decision making when multiple drugs are to be used, which is often the case in cardiovascular diseases. The next five chapters cover the clinical entities of hypertension, angina pectoris, myocardial infarction, heart failure, and cardiac arrhythmias. Each chapter contains one or more algorithms that are very helpful in organizing the decision-making process. The use of new drugs such as angiotensin-converting enzyme inhibitors, tissue plasminogen activators, and inotropes like milrinone (used in Europe and not yet approved by the Food and Drug Administration) are well presented. As the title implies, the book is on drug therapy and fails to discuss other therapeutic modalities such as intraaortic balloon pumps, ventricular assist devices, or coronary artery bypass grafting in the management of cardiogenic shock when drug therapy fails. The next chapter, on cardiac arrest, is a condensation of the American Heart Association's standards and guidelines for cardiopulmonary resuscitation (1986 update). Drug management of infective endocarditis, rheumatic fever, and pericarditis is allocated a brief chapter, followed by a chapter on the treatment of hyperlipidemias. The last chapter deals with antiplatelet, antithrombotic, and thrombolytic agents in the management of ischemic heart disease. Overall this book is useful for medical students, interns, residents, and cardiologists. It can fit in a laboratory coat pocket and provide quick reference to therapeutic options for the medical treatment of cardiac diseases. It is of limited value for the vascular or general surgeon taking care of critically ill patients with problems of hemorrheology, oxygen transport, and hemodynamics; drug therapy of such problems is not well addressed by this book." @default.
