- W2158709968 abstract "to evaluate the differences in heart rate variability (HRV) in mild to moderate hypertensive patients according to day-night decreasing of the systolic (SBP) and diastolic (DBP) blood pressure. 41 patients with mild to moderate essential hypertension (EH) were included in this study (mean SBP/DBP was158.6/101 mmHg, age 49.2±2.1 yr.). All patients were done office BP measurement and bifunctional ambulatory blood pressure (ABP) and ECG monitoring. According to daily index (DI) all patients were divided into 2 groups: 1st gr. – 20 patients with DI for SBP and DBP 10-20 % (“dippers”); 2nd gr. – 21 patients with DI for both SBP and DBP < 10 % (“non-dippers”). Baseline there were not any age, gender or office BP differences between groups. For this study we also assessed the hourly and spectral patterns of the HRV and their correlation with ABP monitoring data. we did not find any significant differences in ABP monitoring data, except higher level of night SBP and DBP in the 2nd gr. Also there we no any significant differences in average HRV patterns between groups. During the correlation analysis we found, that in 1st group was significant direct correlation between ABP data and patterns, that characterized parasympathetic activity: 24-hour, day-time, night-time SBP; night-time DBP and rMSS (r= 0.55, 0.53, 0.62, 0.48 respectively) and between night-time SBP variability and rMSS (r= 0.49, P=0.01) and SDNN (r=0.60, P=0.009) and HRVi (r=0.56, P=0.015). Also we noted direct correlation between night-time SBP/DBP and 24-hour, night LF (r= 0.49/0.53 and 0.49/0.52 respectively); HF (r= 0.54/0.51 and 0.53/0.53), that was evaluated as normal compensative reaction of the vegetative system. In “non-dippers” we found inverted relation between this patterns: 24-hour, night-time SBP and SDNN (r= -0.46, -0.48; P=0.04 and 0.03) and HRVi (r= -0.6, -0.66; P=0.005 and 0.001); night-time DBP and night-time LF (r= -0.48, P=0.04). So as higher was BP as less was HRV and contrary. “non-dipper” patients with mild to moderate EH did not differ from “dippers” by average HRV patterns. But they had more vegetative dysfunction, that characterized by inverted relations between HRV and BP level. In “dippers” this relations were direct and reflected the activation of the compensative mechanisms." @default.
