Despite treatment, 30–50% of patients fail to respond and 15% experience progression to MIBC [1]. According to European Association of Urology guidelines, BCG response is determined by absence of refractory disease after the first or second BCG induction course. Predicting high-risk patients who might not benefit from BCG treatment is pivotal, since delaying radical surgery worsens patient survival [2,3]. Clinicopathologic nor molecular tumor markers have shown consistent results in predicting BCG efficacy [4]. However, a new subgroup of patients with T1G3 disease who require aggressive treatment was recently identified. Characteristics associated with the highest risk are carcinoma in situ, large tumors, and age 70 yr [5]. Besides these parameters, onlymarkers associated with the immune response, such as cytokines, have shown promising results until now [4]. The question remains whether we can predict BCG response. In this issue of European Urology, Kamat et al [6] address this pivotal problem by investigating urinary levels of 12 cytokines before and after the first BCG induction course at 6 wk. From this panel, nine were selected for the cytokine panel for response to intravesical therapy (CyPRIT). The authors used the change in cytokine levels (D) between these measurements, rather than absolute levels during treatment as other investigators have done previously.
